Promotions are periodic increases to game components that run in between events. They generally run for 24 hours at a time.
A premium multiplier is attached to each promotion. By default, everyone has a premium multiplier of x1 for each multiplier type. This x1 premium multiplier is not visible in any of the multiplier summary popups. During a promotion, that promotion's premium multiplier is increased by 1 and it will be visible as a separate line item in the multiplier summary popup.
If you own a premium upgrade for the current promotion, the line item will be labeled "Premium multiplier". If you do not, it will be labeled as "Event multiplier".
There are multiple tiers that can be purchased. If you, for example, own a 2x multiplier, the promotion will be triple (3x) for you during the promotion period.
